Standard Development • OSHA estimates that compliance with the standard prevents about 122 fatalities, 28,400 lost workday injuries, and 31,900 non-lost workday injuries each year. • OSHA estimates that adherence to the requirements of this standard can eliminate nearly 2 percent of all workplace deaths in establishments affected by this rule.

  3. Purpose • Prevents the Unexpected Energization or Start Up of Equipment • Prevents the Accidental Release of Stored Energy • Scope & Application • Applies to Servicing and/or Maintenance • Non-production

  4. Lockout/Tagout Standard does not cover: • cord and plug connected equipment • limited hot tap operations

  5. Service Operations • Such as lubricating, cleaning, or unjamming the equipment may subject the employee to hazards that are not encountered as part of the production operation itself.

  6. Service Operations • Lockout/tagout applies when any of the following conditions occur: • A guard is removed or bypassed • Body contact is made with equipment’s point of operation • The employee is required to place any part of his or her body into a danger zone

  8. Machine Specific Procedures • Specific procedural steps for shutting down, isolating, blocking & securing machines or equip. to control hazardous energy; • Specific procedural steps for the placement, removal & transfer of lockout devices or tagout devices

Machine Specific Procedures • 1910.147(c)(4)(i) "Exception:" The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist:....... • locked out by a single device • exclusive control of the employee • does not create hazards for other employees • No Accidents

  12. Devices • Energy Isolation Devices • Switches • Valves • LOTO Devices • Tags • Locks • Devices that make EIDs lockable • Must be durable capable of withstanding the environment to which they are exposed. • Must be singularly identified. • Must be the only devices used for controlling energy.

  13. Device Requirements • Must NOT be used for other purposes! This includes: • Toolboxes • Lockers • Must be standardized in at least one of the following criteria: • color • shape • size. • Tagout devices must be standardized as to print and format. • Must be identifiable, in that it indicates the identity of the employee applying the devices.

Training Required • Recognition of Hazardous Energy Sources • Types and Magnitude of Energy • LOTO Methods & Means • Limitations of Tags • Retraining Required • change job assignments • change in processes • change in procedures • inadequacies in employee's knowledge • Purpose and use of the energy control procedure. • Procedure and the prohibition relating to attempts to restart or reenergize machines or equipment that are locked out or tagged out.

  16. "Authorized employees" • An employee who performs servicing or maintenance on equipment. • Employees charged with the responsibility for implementing the energy control procedures and performing the service and maintenance. Recognition of applicable hazardous energy sources • Type and magnitude of the energy available in the workplace • Methods and means necessary for energy isolation and control.

  17. Performance & Notification Notification of Employees • "Affected Employees" notified of application and removal • before controls applied • after they are removed
