Improving Community Conditions At Kaiser Permanente, we are working to improve the conditions for health and equity in the community by addressing the root causes of health. Thriving Schools These improvements grow from our collaboration with each community to co-design and co-create solutions that truly make a positive impact.
San Joaquin County 2019 Community Health Needs Assessment March 2019 Collaboration in Action – San Joaquin County • Healthier Community Collaborative – over 20 organizations represented • CHNA/CHIP sub committee – Core Group: Hospitals, Health plans, Public Health • Joint funded CHNA and CHIP • Approach: Health Equity Lens • Outcomes • Accessible/demonstrable priorities (rationale for providers, policy makers, advocates, and funders) • Diverse organizations came together and spurred new relationships, partnerships, and collaboration • Achieve more by aligning resources and scaling • Community Health Improvement Plan • Finding complementary funding opportunities • Upstream funding - built environment in census tracts that demonstrate the highest need
Priority Health Need Selected from CHNA: Healthy Eating Active Living Goal Associated with Health Need: Park Activation/ Beautification Community Health Improvement Plan (CHIP) • OPPORTUNITIES • Childhood Injury Prevention – Update/Fix playgrounds • Obesity Prevention – Zuma, walking clubs • Support for youngsters to join community sports leagues • Nutrition – Farmers’ Market; food demonstrations, picnics • Sense of Community – Community gatherings • Violence Prevention – Address vagrancy and drug use • Economic Vitality – Well-loved park and property values • Safe Routes to Parks – Safer sidewalks, bikeways, roadways to and from park
