    2. A Retrospective Study Done at Madison Family Practice Center (MFPC)

    3. Primary Objective To find out if the CBC ordered by “Providers” at MFPC are indicated .

    4. Secondary Objectives If the “Providers at MFPC are charting effectively to justify the ordered Laboratory test is indicated. If the procedures in place at the MFPC is being followed by the staff.

    5. Materials & Methods Using the Age & Sex Registry at MFPC from the year 2003, the month with the most number of laboratory test was identified. If the laboratory requisition slip indicates that CBC was ordered, these patients were included in the study. Charts of these patients were reviewed.

    6. Charts Reviewed Based on the ff: If CBC is ordered If CBC is indicated at that particular visit If the CBC results are available in the chart, if not computer database was searched for the results If the Chart is readily available

    7. RESULTS There are 116 CBC’s ordered on the Month of September 2003

    8. Number of Charts Readily Available Out of the 116 charts 99 charts were found (99/116) within 30 minutes 85%

    9. CBC results in the Chart Out of the 99 charts available only 69 CBC results were in the chart (69/99) 70%

    10. Reviewing the Computer Database Out of 99 CBC’s ordered 68 were done by Mt. Sinai Laboratory (68/99) 69% Out of the 99 CBC’s ordered 31 were not done by Mt Sinai Laboratory (31/99) 31%

    11. CBC’s Indicated ? Out of the 98 charts reviewed 59 charts showed a very clear indication of CBC ordered as indicated (59/98) 60%

    12. CBC’s not Indicated ? Out of 98 charts reviewed 3 charts showed that the diagnosis on the particular visit is not supportive of ordering a CBC (3/98) 3%

    13. No Indication in the Charts Out of 98 charts reviewed 36 charts showed no clear indication in the reviewed charts (36/98) 36%

    14. Recommendations A better system in filing and retrieving laboratory results or follow the system in place. Our charts should reflect a clear indication of the reason/s why we are ordering CBC’s or Do a good problem oriented notes
