  2. The Internship Process • Choose a site • Get approval from your advisor • Complete the application • Add the credits with and ADD/DROP form • Keep a weekly log of your hours • Submit your summary report and evaluation

  3. Choosing a site • Start early! Find an organization in the community health field doing work that aligns with your goals. Contact the organization about a possible internship position. • Visit our website for a list of potential organizations. • Visit the SCH office (URBN 450) to look at the binder of internships recently completed by fellow students. • Once you have an opportunity lined up make an appointment with an advisor. If you are short on time, email them immediately.

  4. Get approval from your advisor • You must meet with your advisor the term prior to the one you would like to begin your internship in • If you want to start your internship in spring, see your advisor in winter • In rare cases you might need approval quicker. In this case, email your advisor as soon as possible with the following information: • Name and ID# • Name of internship site and supervisor • Your responsibilities as an intern • With your advisor, determine the amount of credits you will be registering for

  5. Complete the application • This can be found on our website • It is important to fill out the application completely! • Get your worksite supervisor’s signature

  6. Add the credits with ADD/DROP form • You cannot register online! • You must either: • Bring in the email from your advisor approving your internship • Bring in the pink slip given to you by your advisor approving the internship • Get your ADD/DROP form (available in URBN 450 or Neuberger Hall) stamped by advisor or office assistant by the end of the first week of classes • Take your form to Neuberger with a valid student ID

  7. Keep a weekly log of your hours • Include the days and times you spent at your site • Include a short description of the activities you did • You can keep an electronic version of this or submit a handwritten (legible) version

  8. Submit your summary report & evaluation • Complete sections I & II of the summary report, which can be found in the internship packet on our website • Submit the evaluation to your worksite supervisor. Ask them to fill it out and send it to your advisor • Fill out the optional follow-up survey

  9. Special Situations • You can do your internship at multiple sites. For each new site, you must get approval and submit a new application • You can pay for the credits over multiple terms. If you finish your internship in one term but pay for it in other terms, you need to email your advisor each term that you are registered to get the grade for that term • You can do the internship over multiple terms. You must email a progress report to your advisor at the end of each term to get a grade if you are not turning in a summary report
