  2. HAVE MATERIALS READY • Tape • Signs • Gloves • Cups • Labels • DD2624 • Unit ledger • Alpha roster • Observer Statements

  3. BRIEF THE OBSERVERS AND HAVE THEM SIGN OBSERVER STATEMENTS

  4. THE SOLDIER APPROACHES THE STATION • Have the Soldier remove their ACU top • Make sure the Soldier has an ID card • If there is no ID card, verify the ID of a soldier in another way

  5. SOLDIER VERIFIES INFORMATION • Have the soldier look at their personal information on the DD 2624, unit roster, and bottle label • If the information is incorrect, draw a line through the incorrect information, write the correct information, initial, and date

  7. THE DONOR INITIALS THE BOTTLE LABEL, MAKING SURE TO DO IT PROPERLY (No signatures, Jr., or II)THE UPL PUTS THE LABEL ON THE BOTTLE IN VIEW OF THE DONOR

  8. REMOVE THE BOTTLE FROM THE BOXAND REPLACE IT WITH THE SOLDIER’S ID CARD

  9. SPECIMEN IS COLLECTED AND BROUGHT BACK TO THE COLLECTION STATION

  10. UPL TAKES THE BOTTLE • Makes sure the cap is tight • Makes sure the specimen has 30ml • Makes sure the specimen is not adulterated • If adulteration is suspected, have the Soldier stand fast and notify the commander.

  11. THEN, THE UPL… Puts tape on the bottle Initials the bottle

  12. THE UPL REMOVES THE ID CARD FROM THE BOX AND RETURNS THE BOTTLE

  13. FINAL ACTIONS • The Observer signs the ledger • The Donor signs the ledger • The UPL Returns the ID Card to the donor. • It is good to go through the box at the end of the process to verify that all the bottles have been taped, are in the correct slot, and that all have been initialed by the donor and UPL
