  1. NDIS Community Nursing Services P home care is a registered NDIS provider who assists, helping and supporting you on NDIS journey. NDIS community nursing involves assessing and addressing the healthcare needs of individuals, families, and communities within their own environment. The National Disability Insurance Scheme (NDIS) provides funding and support to people with disabilities in Australia. The scheme is designed to provide assistance to people with disabilities so that they can lead independent lives and participate fully in their communities. The NDIS offers a range of support services, including community assistance. Community assistance is aimed at helping people with disabilities to access and participate in community activities, events, and programs. This may include assistance with transport, support to attend social events or groups, or help with developing skills that will enable them to participate more fully in their communities. To access community assistance under the NDIS, individuals need to meet the eligibility criteria and go through the assessment and planning process. This process involves working with an NDIS planner or Local Area Coordinator (LAC) to identify the person’s goals and support needs. Once the person’s plan is developed, it will include funding for community assistance if it is identified as a need. The NDIS provides funding for community assistance through a range of service providers, including disability support organisations and community groups. These providers can assist individuals with accessing and participating in community activities, events, and programs. If you or someone you know requires community assistance under the NDIS, it is recommended that you contact the NDIS or a local disability support organisation for more information on how to access these services. The benefits of community nursing services under the NDIS include:

  2. 1. Improved health outcomes: Community nursing services can help people with disabilities manage their health conditions and reduce the risk of complications. This can lead to improved health outcomes and a better quality of life. 2. Improved access to care: Community nursing services can provide care and support for people with disabilities who have difficulty accessing medical care in a hospital or clinic setting. 3. Individualized care: Community nursing services are tailored to meet the specific needs and goals of each person with a disability. This can ensure that they receive the care and support they need to live their life as independently as possible. 4. Increased independence: Community nursing services can help people with disabilities maintain their independence and live in their own home or community setting. 5. Reduced hospital admissions: Community nursing services can help reduce the need for hospital admissions, which can be stressful and disruptive for people with disabilities and their families. As an approved NDIS Provider Social and recreation activities can include things like: ○visiting your friends and family ○active hobbies, such as bike riding, skiing or kayaking ○playing sport, such as tennis, surfing or basketball ○going out, for instance, to the movies or a concert ○going places for fun, such as shopping or visiting a museum ○relaxing, like meditation or yoga ○learning new skills, like dance, art classes or quilting. You will need to pay for the costs of the activity that everyone would pay for such as membership or entrance costs. We may then be able to fund the support you need to take part in the activity because of your disability.
