정의 & 역학 • 비중격이 휘어져 코와 관련된 증상을 일으키거나 코막힘, 부비동염 등의 기능적 장애를 유발하는 질환 • 동, 서양인을 합한 전반적 발생률은 약 68% • 한국인에서 비중격 기형의 유병률은 22.38%로 보고 • 남 24.24%, 여 19.80% Page 16

진단 • Anterior rhinoscopy • 혈관수축제로 비점막을 수축시켜서 비강 내부를 좀더 잘 볼 수 있다. • Cottle test • 환자의 뺨이나 비익(ala)을 외측으로 당겨서 코막힘이 나아지는 지 확인 Page 19

치료 • 증상이 없거나 합병증, 기능적 장애가 없으면 치료는 불필요함 • 비점막 혈관수축제의 경구 복용 • 생리식염수의 비강 분무 • 비전정과 비판 부위의 안연고 도포 • 수술적 치료 • Submucous resection of the septum • Morselization (분쇄법) • Septoplasty Page 20

치료 • 수술의 적응증 • 비중격기형으로 인한 코막힘, 재발성 비출혈, 두통, 때로는 코골이와 같은 증상이나 비강내 기능적 장애가 있을 때 • 다른 비내 수술을 방해하는 요인이 될 때 • 만성 비-부비동염과 같은 주위기관의 합병증을 유발할 수 있을 때 • 다른 부위의 수술에 필요한 연골이나 골편을 얻을 때 • 비중격을 통하여 뇌하수체 적출술을 시행할 때 • 기능적 또는 미용적 외비성형술의 동반수술로서 시행할 때 • 증상을 동반한 비중격 천공일 때 Page 21
