  4. Specific (and strategic): Linked to your personal health goal. What aspect of health would you like to improve upon. Answers the question— • Who? • What? SMART Not: I will lose weight Yes: I will lose 45 pounds S

  5. Measurable: The success toward meeting the goal can be measured. Answers the question— • How? SMART Not: I will lose weight each week Yes: I plan on losing 1-2 pounds of weight ever week. If I do not reach my weekly goal I must make corrections to my next week goal. I will use a chart to monitor my progress. M

  6. Attainable: Goals are realistic and can be achieved in a specific amount of time and are reasonable. SMART Not: I will lose 8-10 pounds a week. Yes: I will lose at least 1 pound a week. A

  7. Relevant (results oriented): The goals are aligned with current tasks and projects and focus in one defined area; include the expected result. SMART Not: I will read every night to keep my mind off of eating. Yes: I will monitor my caloric intake and caloric expenditure every day to make sure I am on target for losing at least one pound per week. I will also exercise 4 times a week for 30 minutes at 60% of my max heart rate. R

  8. Time framed: Goals have a clearly defined time-frame including a target or deadline date. SMART Not: I will reach my goal at the end. Yes: Start date 1/1/15 End date 1/30/16 T
