    2. Role of LA County Homeless Coordinator Oversight of the Homeless Prevention Initiative (HPI) funding. Liaison and consultation with the five LA County Board Offices around issues related to addressing homelessness. Work with communities to promote the implementation of housing with supportive services for various homeless populations. Focus on ending chronic and veteran homelessness. Work with the County Departments to contribute supportive services and other resources to housing efforts for homeless individuals and families

    3. Permanent Housing With Supportive Services (PHSS) Solution to homelessness from a cost and social standpoint. The level of services and the particular model will depend on the particular target population. Chronic homeless Families with significant challenges TAY Temporarily economically challenged

    4. Permanent Housing With Supportive Services, Cont. Housing Types Project-Based – A building with services offered on and off site depending on the housing model and each resident’s particular needs. May be a new development or attrition in an existing program. Scattered-Site – Using existing market rate rentals with services either offered by a mobile team or off site depending on the housing model and each resident’s particular needs.

    5. Components of PHSS Housing structure (project-based building, market rate rental) Rental subsidy to make it affordable (S+C, Section 8) Supportive services (housing case management, health, mental health)

    6. How to Implement PHSS in Communities Review of assets of the geographic area of the particular community. Housing Authority(s) Existing project-based housing programs Housing developer for new projects Supportive services providers

    7. How to Implement PHSS in Communities, Cont. Involve important stake holders/Partners including: Service providers (non-profits, FQHCs, mental health, substance abuse) Business community Housing developers Collaboratives/consortiums Community members/public Consumers Technical assistance (CSH, etc.) Plan for different target populations Right person, right place, right time Tracking of data
