Combination of Didactic/Clinical Hours 100 Plus Clinical Hours – Saint Alphonsus Eagle Emergency Department, St Luke’s Eagle Urgent Care, St Luke’s Pulmonary Associates, St Luke’s Idaho Cardiology Associates, Saint Alphonsus Neurology Department, Health and Welfare Mobile Crisis Unit, Terry Reilly Health Services, St Luke’s Children’s Hospital.

  4. Ada County Community Paramedic Program Mobile Seasonal Flu Vaccination Program for all County employees. Increased Vaccination Rates by 12%. System Wide At Risk Patient Referral/Education/Care Coordination Program. In-Home Patient Follow-Up with Post Hospital Discharged Patients • In home visit, 30-day follow • Collaboration, Care Coordination In System 911 Community Paramedics – High/Low Acuity Patient Type. $4000 Costs Savings in Two Months. Collaboration with APS, MCU

Pilots Medical Clearance of Mental Holds in the field • Evaluate, PD transport to psych facility • ED savings Hypertension Identification and Care Coordination. • Treat and Release Patients • Phone call follow up • In home re-evaluation of HTN, care coordination In-Home Follow Up/Care Coordination with ED Discharged Fall Patients • In home follow up and care coordination with identified fall patient from Senior ED.

  6. PROOF OF CONCEPT …OR HOW I LEARNED TO STOP WORRYING AND LOVE EXCEL -What are we looking for? -How do we keep track of data? -Standardization of data quality -ePCR vs. surveys vs. Excel -Output: Research? Evaluation?
