Community PE Service Pathway - Anticoagulation Registered GP referral to Secondary Care • Diagnosis of PE by Secondary Care • NBT initiates patient on Warfarin (Day 1) • NBT assesses patient for Community Anticoagulation Patients not suitable for community anticoagulation Patients suitable for community anticoagulation: minimal symptoms (i.e. not requiring opiates/oxygen etc); physiologically stable; TNI/TNT normal; and imaging which shows a small to medium clot load • NBT to refer patient to GP Care PE Service via Telephone Referral on Day 1 (or next working day if Day 1 is a weekend/bank holiday) • NBT to fax TTA to GP Care PE Service on same day as the Telephone Referral (to include patient contact details) • GP Care office to telephone DVT Treatment Centre to ascertain next day appointment time • GP Care office to telephone patient to advise next day appointment time • GP Care office to initiate Patient Record Card, attach TTA and fax to DVT Treatment Centre • DVT TC to continue anticoagulation next day according to protocols already used within the GP Care DVT Service Discharge: Patient experiences increasing pain or breathlessness Discharge: Target INR is achieved Secondary care (NBT Ward 105/C) To continue provision of care within an acute setting Registered GP To investigate secondary causes and register with Haematology for INR testing Key: GP Care PE Service Interfaced Services
