Focus on Person-Centered Planning • Process-oriented approach to empowering people with disability labels • Focuses on people and their needs • Individual is empowered to determine the direction for his/her life, not on the systems that may or may not be available • Ultimately leads to greater inclusion as valued members of both community and society
Vision LifePrep@Naz will: Provide opportunities for individuals with intellectual disabilities to continue in their education and development with peers on a college campus Influence the collaborative communities that come together by: Raising the level of expectation for individuals with intellectual disabilities Broadening the construct of diversity to include ability Preparing each participant to obtain meaningful employment
Mission The mission of LifePrep@Naz is to provide community learning experiences that allow for the transformation of individuals with intellectual disabilities from students to self-determined contributors in our society

Year One Course of Study - sample • CCC 101 – Functional College Communication I (3 credits) • FSC 1XX – Functional Skills Elective (3 credit) • PED XXX – Health and wellness • FYS 101 – Freshman Seminar + Lab (3 credits) • VOC 101 – Vocational Experiences (3 credits) • CCC 102 – Functional College Communication II (3 credits) • FSC 1XX – Functional Skills Elective (3 credit) • PED XXX – Health and wellness • SER 101 – Service Learning (3 credits) • NAZ XXX – Nazareth Elective (3 credits) • VOC 102 – Vocational Experience (3 credits) Semester 1 (12-15 credits) Semester 2 (12 -15 credits)
Non-resident participation • LifePrep@Naz is designed to facilitate the participation of neighboring school districts • 15:1 Special Class, including some adult ARC clients
How do we prepare our students? Within a PCP framework: • Academic rigor • Independence • Ability to work with a variety of people • High levels of self-advocacy • Access to electives based on student selection
