  2. Annual Public Health Report • Theme : ‘Choosing Health : Making healthy choices easier’ • Impact of our lifestyles on future life expectancy • Health is inextricably linked to way people live their lives and opportunities to choose health

  3. The Local Population • GP registered population 178,000 • ↑ over 65 yrs old • ↑ by 5.49% in 15yrs • Ageing population

  4. The Local Population • Low Infant mortality rate • Life expectancy is increasing (2001-2003) • Deaths from heart disease/ stroke and cancer are decreasing

The Local Population • 67% report health as ‘good’ compared with 68.8% in E&W • Poor uptake of breast (67%) and cervical screening (73%) • Links between health and deprivation • 10 out of 17 wards are in 4th/5th quintiles when ranked by IMD score • 5 of 107 Super Output Areas in 10% most deprived in England • 1 is in worst 3% : Neighbourhood Management Initiative funding

  6. Smoking • The leading cause of preventable illness and premature death • Prevalence in Southend 28% (26% EoE) • An estimated 200 people die every year in Southend from smoking related illnesses • Many people took up smoking as a teenager – smoking is highest in young adults

  7. Second hand smoke • Contains regulated hazardous air pollutants including 50 known or suspected cancer-causing agents • Exposure of non-smokers: 24% ↑risk of lung cancer, 23%↑risk of heart disease • Timetable for smoke free places: • End 2006: all NHS and government departments • End 2007: enclosed public places & workplaces • End 2008: Arrangements for licensed premises

Smoking : Local Action • PCTs given 4 week quit targets • 2937 people stopped smoking with help of NHS in the last 3 yrs • GPs / South Essex Stop Smoking Service • Community pharmacists: NRT Pilot • Specialist help for pregnant smokers • Prevention: National Curriculum • Included in healthy schools standards

Tobacco Control • Underage sales : Trading Standards • Local Public Service Agreement includes target on tobacco control • Smokes free places project co-ordinator & Environmental Health Officers • National clean air awards to local businesses (childminders) • Smoke free environment helps to reduce amount people smoke – or stop and also helps prevent young people from starting

Obesity • Declared as a ‘global epidemic’ • National prevalence trebled since 1980’s • In 2003 England 66% men and 50% women overweight or obese • 22% boys and 27% girls aged 2-15 years are overweight or obese • Can cause serious illness – heart disease, cancers, high blood pressure • Reduces life expectancy by up to 9 years

Prevention of Obesity • Imbalance food intake vs. energy expenditure • Importance of economic, environmental, social and cultural factors • Aim: physical activity as part of everyday life • Important role of local authorities: provision of cycle lanes, road safety measures, ‘safer journeys to school’ – walking bus • Health walks and exercise referral • School sports co-ordinator partnership

Prevention of Obesity • Unhealthy diets – cancer, heart disease and obesity • 5 portions of fruit & veg a day ↓ risk • Free fruit scheme in schools for 4- 6yr olds • Diet & nutrition as part of healthy schools • School meals: role of EHOs in food safety and nutritional content • Promote good eating habits from childhood – work on breastfeeding and weaning policies • Obesity strategy developed • Getting information on obesity – schools & GPs

Sexual Health • An important part of physical and mental health • Indicators: rates of sexually transmitted infections and teenage pregnancy • England has highest teenage conception rate in developed world • Southend 50.4 per 1000 girls under 18 (higher than England, EoE & Essex) • 13.8 % reduction in teenage pregnancy in Southend since 1998

  15. Sexually transmitted infections • Reflects national picture • Rates of STIs ↑ • 1 in 10 young women has chlamydia • Increasing number of cases of HIV • Consequences of STIs

Sexual Health : Local Action • Multiagency Teenage Pregnancy Board –oversees strategy • Teenage Pregnancy Co-ordinator : • Work with foster carers on sexual health • Multiagency SE Essex Sexual Health & HIV Strategy Group • PCT appointed African Communities Worker • Southend: pilot site for Chlamydia screening • Training for teachers on sex & relationships education (SRE)

  17. Sexual Health : Local Action • Developed framework for sexual health strategy • Modernise service and increase access • Increase uptake of chlamydia screening to meet national target in 2007 • Raise awareness : national and local campaigns

  18. Alcohol • Alcohol causally related to 60+ medical conditions – most detrimentally • 150,000 hospital admissions per year in England are as a result of alcohol • Costs NHS an estimated £1.7 billion /yr • 7% of adult population are dependent on alcohol

  19. Alcohol • Concerns about drinking patterns of young people • Binge drinking – highest in 16-24 yr old • Association with crime and disorder, particularly anti-social behaviour • Vulnerability of individuals when drunk • Led to publication of an Alcohol Harm Reduction Strategy for England

  20. Alcohol: Local Action • Provide information : sensible drinking message • Alcohol education is statutory requirement of National Curriculum • Part of PSHE in schools • Underage sales : Police / Trading Standards -proof of ID card • Licensing Act 2003: cumulative impact policy

  21. Alcohol : Local Action • Multiagency group developed a Southend Alcohol Harm Reduction Strategy • Proposed local enhanced service with GPs for detection of alcohol abuse and provision of brief intervention • Need to improve local knowledge on impact of alcohol

  22. Health Protection • Importance of close working relationship between the PCT and local authorities : • Infectious diseases • Emergency Planning • South East Essex Emergency Planning Forum • Poor uptake of immunisations • MMR catch up in schools to stop mumps epidemic
