Across West Yorkshire... How will it work on the ground ? • For clients who are not engaged with any existing services / projects • Must meet the requirements of the Chaos Index and MEAM • Must have a minimum of 3 out of the 4 needs • Service to supplement and support existing projects not to replace or duplicate them • Close links with all existing providers and partners • ‘Buy in’ from commissioners and stakeholders
How will it work on the ground ? • Staffing: • Recruitment on going with start date of June 1st • First referrals mid / late July
Numbers of clients : Year 1: Lead Navigator - 5 Navigator - 10 Year 2- 5 per year: Lead Navigator - 8 Navigators - 60 Year 6: Lead Navigator - 5 Navigator - 10 302 clients over the 6 years
Input from partners • Referral process / operational delivery - Working closely with all existing providers • Locality Group - Operational managers to look at difficult cases and review the service • Multi – agency review board - Key ‘difficult’ cases taken to the board made up of key commissioners to look at solutions and system change
