This year’s flu vaccine program If you get an annual TB test: • You are required to either get the flu vaccineorwear a surgical or isolation mask during flu season for all face-to-face patient interactions or to enter a patient’s room for any reason. • If you decline the flu vaccine, you must sign a declination form and give the reason why (medical/religious/other specific reason).

This year’s flu vaccine program If you don’t get an annual TB test: • You are strongly encouraged to get vaccinated against the flu! • If you don’t: You must wear a surgical or isolation mask during flu season for all face-to-face patient interactions, or to enter a patient’s room for any reason.

Where can I get the flu vaccine? • Walk-in clinics • See Health System Headlines or www.med.umich.edu/u/flufor a schedule • Inpatient floors • See your “Vaccination champion” • Ambulatory Care sites • From a member of the clinical team • Occupational Health - Med Inn (during business hours) • Your own provider, or a pharmacy or store • Fax proof to Occupational Health at 763-7405 to record your vaccination

If you are not vaccinated… • Once the flu arrives in our area, a “masks on” message will go out. • From then until the end of flu season, all unvaccinatedemployees, students, volunteers, contractors & vendors must wear a surgical or isolation mask during face-to-face patient interactions. • This includes entering patient rooms for any job-relatedreason (not just patient care). • Masks will be provided.
