For 16 and 17 year olds, who left K12 schooling without a diploma • Used to request permission to: • enroll in the program • take the GED® Test Enrollment Application
Fill-out the first page • Parent/Guardian signature (some exceptions) • Acquire a withdrawal form or letter from last school (some exceptions) • Present photo ID confirming age Enrollment Steps for Students
Parent/Guardian completes form in the presence of a witness • printed name • signature • date Parent/Guardian Permission
Married Emancipated Court Ordered/Adjudicated Enrolled in a Special Program *with documentation Parent/Guardian Exemptions*
Withdrawal form from last school attended • Letter from the superintendent verifying the applicant • is no longer enrolled or • was never enrolled Withdrawal Form or Letter
Used in place of a withdrawal form for Home School Students only Completed by the Parent/Guardian Must be notarized Home School Affidavit
Three statements • Parent/Legal Guardian declaration • Applicant has not graduated • School System has been notified of the student’s withdrawal from home school Home School Affidavit, cont.
Court Order/Adjudication • Juvenile Justice • Special Program enrollment • Job Corps or Youth Challenge *with documentation Withdrawal Form Substitutions*
Submits application and documentation • Shows Picture ID • name • Address • DOB • signature • photograph • Program Admin approves Next Steps
Attend class for at least 40 hours Score at least 9.0 on the D or A level TABE in Reading, Math, and Language Then, take the GED Ready™ practice test GED® Testing Approval Steps • Score at least a 158 on all four parts • Schedule a time with their teacher to register • Agree to maintain enrollment until they successfully complete testing
Work with students to complete Steps 1 and 2 of GED Registration online or by phone • www.ged.com or 1-877-EXAM-GED • Teacher recommends the student and program administrator gives final approval • Program submits Request for GED Testing Approval to Georgia GED Testing Program • Approval takes about 2 business days and the student will be notified by email GED® Testing
