Policy - course of action that guides present and future decisions. The action is based on given conditions and selected from among identified alternatives. • Policy decisions often establish the direction for future trends that impact informatics • Healthcare policy - established on local, state and national levels to guide the implementation of solutions for the populations health needs
Healthcare Policy and NI as specialty • 1. Recognized as a specialty by ANA in 1992 but failed. • 2. Combination of nursing, information and computer science to manage and process nursing data into information and knowledge for use in nursing practice. • 3. Facilitates in the integration of data, information and knowledge to support patients, nurses and other providers in their decision making in all roles and settings. • 4. Its domain focus on data and its structures, information management and the technology, including databases, needed to manage information effectively. • 5. Significant use of theory from linguistics, decision science, cognitive science, communication, engineering, library science and organizational dynamics.
Interdisciplinary and Differentiated Practice • * Informatics nurses should assist with the development and implementation of technology tools for clinical practice, evaluation of the effectiveness of technological tools on nurse’s work and help prepare nurses to use technologies.
Preparation for Specialty Practice • 1.Educational Programs • - DN and HRSA funded two Masters program for NI programs. • 2. Certification Examination • - Available through the American Nurses Credentialing Center • 3. Research Focus • - Scientific study • 4. Nursing and Multidisciplinary organizations
Events influencing Informatics • I. Nursing shortage • II. A concern for patient safety • NAHIT – a partnership of diverse healthcare leaders who are working to influence the use of technology to improve patient safety, quality and efficiency
III. National informatics initiative • HIPAA • National Agenda for Nursing Informatics • IV. Delivery of service through telehealth
I. Nursing Shortage • Reasons : • - Availability of more attractive career opportunities for women • - Decreased interest in nursing as a career • - Higher ratio of older associate degree rather than baccalaureate counterparts • - Difficult work environments • - Older nursing workforce • II. Patient Safety • - Implementation of computerized physician order entry • - Connecting for Health (CFH)- another public-private collaborative, was established by the Markle foundation with additional support from the Robert Wood Johnson Foundation. • - The Markle Foundation is a private philantrophy that works to enable technology to improve lives.
III. Nursing Informatics Initiative • Driving forces for the increased clinical automation • Patient’s safety • Escalating healthcare cost • Availability of improved and cheaper CIS
Incentives for the Use of Health IT and Establishing the Position of National Health Information Technology Coordinator • Components of the order: • Establish national health info. Technology coordinator position • Work to develop a nationwide interoperable health IT infrastructure • Develop, maintain and direct implementation of a strategic plan to guide implementation of interoperable health IT both in public and private sectors.
* National Health Information Infrastructure(NHII) • A voluntary initiative, involving a three-stage process, and is intended to improved the effectiveness, efficiency and overall quality of health and healthcare in the US. • * Health Insurance Portability and Accountability Act (HIPAA) • Improve public and private health programs by establishing standards to facilitate the efficient transmission of electronic health information. • * National Agenda for Nursing Informatics • DN, HRSA – responsible for setting the national policy to guide the preparation of the nursing workforce, including the preparation of the area of NI.
DN convened the National Nursing Informatics Workgroup (NNIWG) to make recommendations to the National Advisory Council for Nurse Education and Practice (NACNEP)
Five key directions for informatics in nursing education and practice • Prepare nursing faculty in informatics • Increase collaborative efforts in NI • Educate nursing students and practicing nurse in core informatics content. • Prepare nurses for specialized skills in informatics • Enhance nursing practice and education through informatics project
NIA Strategic direction for NI • To enhance nursing practice and education through informatics projects. • To improve faculty skills in NI • To include core informatics knowledge and skills in all undergraduate, graduate and continuing education programs. • To increase the number of nurses with specialized skills in informatics • To increase collaborative efforts in NI
Telehealth • Use of electronic information and telecommunications technologies to support long-distance clinical healthcare, patient and professional health-related education, public health and health administration
