  13. Under fives extension • Runs from 8th Dec 2009 to 31st March 2010 • All children over 6 months and under 5 years • Dob range 8th Jun 2004 to 31st October 2009

  14. Vaccines and doses • Preferred: GSK Pandemrix • One dose of 0.25ml for those under 9y unless immunosuppressed • Optional for egg allergy/mothers preference: Baxter Celvapan • Two doses of 0.5 ml for all ages: three weeks between each dose
