5. SBE Sub-Contracting Goals • SBE goal • This solicitation is open to all firms • No SBE documents due at time of SOQ submittal • SBE documents will be due to EOD at time of each JOA

6. What is Job Order Contracting? • Alternative Project Delivery Method (APDM) • This JOC is a 2-Step Qualifications-Based Selection that includes a cost value element • Evaluation of Qualifications and Price Based • Not a Bid • No guarantee of work • Projects as they come up – “As needed” basis • No Federal Funding

7. Procurement Description • City of Phoenix is seeking qualified Job Order Contractors on an “on-call” basis • Up to 2 contractors may be selected • Performance & Payment Bonds required for each Job Order Agreement • No interviews – 2nd step is RFP process (selecting 4 contractors to move forward) • Job order agreements performed under this Contract may be executed for up to $2,000,000 each in construction costs • JOC for 36-month duration or complete utilization of initial contract value w/ one 24-month option to extend and increase capacity

8. Scope Of Work • Removal and replacement of non ADA compliant sidewalk curb ramps as well as deteriorated or damaged concrete curb, gutter, valley gutters with and/or without aprons, and sidewalk. • Installation of new sidewalk and ADA compliant sidewalk curb ramps in accordance with MAG and/or Phoenix Supplement to MAG and/or field designed to retrofit existing conditions within the dedicated public right-of-way on Major Arterial, Collector, Local and Industrial streets within Phoenix city limits, as detailed below.

9. Scope Of Work At a minimum, the JOC Contractor will be expected to successfully perform the following construction services, including but not limited to: • Set up and take down traffic control devices in accordance with the City of Phoenix Traffic Barricade Manual. • Excavate, prepare subgrade, set concrete forms to match existing grade and drainage elevations, pour and finish concrete valley gutters with and/or without aprons, curb and gutter, alley and driveway entrances and sidewalk to match existing elevations and drainage. • Excavate, prepare subgrade, form to fit field conditions, pour and finish ADA compliant concrete sidewalk ramps, including the installation of detectable warning. • Replace asphalt concrete pavement to match existing asphalt pavement and new concrete. • Adjust utility junction boxes, man and hand hole covers. • Other services as may be required, incidental to this JOC.

Statement of Qualifications Evaluation Criteria • General Information – (15 pts.) • Experience and Qualifications of the Firm – (25 pts.) • Experience of Key Personnel Assigned – (20 pts.) • Approach to Performing Required Services – (30 pts.) • Overall Evaluation of Firm and Ability – (10 pts.) (No response required for ‘E’ – Panel Evaluation) • Put in order as listed! • Be complete, but concise!

Submittal Requirements Refer to Section VI of the RFQ for a detailed description of the submittal requirements. • Cover Letter – One (1) page • Evaluation Criteria – 12 pages What will be Counted Each page side with criteria information, pages with photos, charts, and graphs What is Not Counted Cover letter, Table of Contents, plain tab dividers, front / back covers that have pictures without descriptive captions

12. Statement of Qualifications Submittal • Submit 7 copies of the SOQ • SOQ Due: 12:00 Noon Arizona time Friday, February 10, 2017 Location: Phoenix City Hall 200 W. Washington 6th Floor, Submittal Box • Include firm name and procurement title on submittal cover

13. Procurement Schedule Pre-submittal Meeting January 30, 2017 SOQ due at noon February 10, 2017 Selection (1st step) February 28, 2017 RFP Phase (2nd step) March, 2017

14. Grounds For Disqualification • Receipt of SOQ after submittal date or time • Less than 7 copies of SOQ • Deposit of SOQ in wrong location • Violating “Contact with City Employees” policy • Failure to provide Bonding Statement
