Automatic Digital PE Wear Measurement for Total Hip Arthroplasty

  1. Abstract [Background] Quantifying the amount of wear is important for total hip arthroplasty because we can predict the prognosis early. Many digital methods have be reported, none of them are fully automatic. Using the cross correlation and edge detection operators, we developed a fully automatic program which can automatically recognize the prosthesis and measure the wearing. [Methods] We simulated 64 hip X-rays using simulator, including two different abduction angles, two different anteversion angles, four different superior wearings, and four medial wearings. All 64 wearings are measured by our method. We also collect 88 real hip X-rays. The wearing of the 88 X-rays are measured by Livermore’s method and our method. Then we compare the detection rate. [Results] With simulated radiographs,only 3 misdetections are found. The error of medial wear is -0.023±0.176mm. The error of upper wear is -0.152± 0.315mm. For real X-rays, detection rate of our method is 53.4% (47/88). The detection rate of Livermore’s method without aid is 23.9% (21/88). The detection rate of Livermore’s method with aid is 78.4% (69/88).   [Conclusions] We designed a program which can automatically measure polyethylene wear. We have tested it on simulated X-rays and the result. The future work is to increase the detection rate and clinical validation.
