Progress update • Prototype ‘live’ from 20th November within Hebburn • Phase II roll out to Jarrow 1st April 2015 • Continued to review international best practice- New Zealand delegation • Completion of a number of organisational Development meetings • Internal referral processes • Internal communication processes • Integration of delivery team II • Local, regional and national recognition

  4. Evaluation metrics • Patient/carer experience • Staff satisfaction (survey) • GP satisfaction (survey) • No of quantitative measures: • Reduced reliance on hospital • Reduced number of contacts

  5. How satisfied are you with the current model of service delivery for community nursing and social care teams? 0 = extremely unsatisfied , 10 =extremely satisfied

  6. To what extent does the current service deliver seamless patient/client care? 0= extremely fragmented, 10 = extremely seamless

  7. How would you rate morale within your current team? 0= extremely low, 10= extremely high
