Progress update • Prototype ‘live’ from 20th November within Hebburn • Phase II roll out to Jarrow 1st April 2015 • Continued to review international best practice- New Zealand delegation • Completion of a number of organisational Development meetings • Internal referral processes • Internal communication processes • Integration of delivery team II • Local, regional and national recognition
Evaluation metrics • Patient/carer experience • Staff satisfaction (survey) • GP satisfaction (survey) • No of quantitative measures: • Reduced reliance on hospital • Reduced number of contacts

Next steps • Implement borough wide roll out in summer 2015 • Identify additional opportunities for expanding the model • Continued to monitor impact • Promote our work and raise awareness of South Tyneside's success!
