Dilaceration • Dilaceration is a crown or root showing angular distortion that occurs during root development. • It is caused by a deformity of HERS after the crown is completely shaped due to an injury or pressure. • HERS or Hertwig’sEpithileal Root Sheath functions to shape the roots and induces root dentin formation. • The function of this sheath or membrane is to shape the root(s) by inducing dentin formation in the root area so that it is continuous with coronal dentin. • Dilaceration can occur in any tooth or group of teeth during tooth formation.
Accessory Roots Accessory roots or supernumerary roots may be due to trauma, pressure or metabolic disease that also affects HERS. Any tooth may be affected, but it occurs mainly with the permanent third molars, mandibular canines and premolars.
Role of the Dental Team • Dilaceration and accessory roots can cause problems during extraction and endodontic treatments. • With dilaceration, there can be sharp bends that act as a hook, lodging the tooth to the jaw. They usually fracture during the extraction and require surgical removal. • The endodontic treatments can be very challenging due to the difficulty to access the apex. • Preoperative radiographic examination is necessary to rule out these disturbances.
