2. What is CQUIN? ‘A payment framework which is designed to incentivise providers into achieving goals around the following quality domains: Effectiveness, Safety, Innovation and Experience’

3. Last Year’s Scheme • Homes needed to complete 5/6 indicators successfully • Payments were based on a % of the homes contract value from the previous year. • 38 Homes successfully completed the CQUIN scheme in 2010/11 (32% of the care homes market) • Recognition from other local authorities.

7. This Year’s Scheme • It’s a ‘one off’ voluntary incentive scheme • Smaller scale than last year’s CQUIN • 2 gateway requirements • 5 CQUIN indicators • Successful homes will receive incentive payments and a certificate of achievement

8. Our Overall Aim • Approximately 197 Fractured Neck of Femur (NOF’s) procedures in Torbay • 25% of these are estimated to be from Care Home Residents. (49 procedures) • With your help we aim to drive down the number • of Fractured Neck of Femurs (NOF’s) from Care Homes, • saving pain and saving lives.

9. CQUIN Payments • All Homes must meet indicator 4. • Other indicators can be achieved and will be paid independent of one another.

10. Implementation of the Scheme How does it work? • Use the ‘CQUIN Indicators 2011-12’ Document as your guide to completing the scheme. • Implementation from December 2011 to April 2012. • A panel of specialists will assess the evidence supplied by the homes. • The Trust will notify the homes of the outcome following assessment.

13. Gateway Requirements

14. Gateway Requirement 1 (G1) • Improvements in emergency planning • Better access to vacancy information What is Required? All Homes are required: • supply contact details for the manager and a nominated individual for the home in their absence. • ensure that these named individuals can access and report vacancy levels at all times.

15. Gateway Requirement 2 (G2) • Commit to the seasonal flu vaccination programme for the protection of staff and residents. What is Required? Homes are required to: • supply a list of the staff who have been vaccinated and where possible the date of the vaccination. • provide details of how they have engaged the staff and communicated the benefits of them having the vaccination to them. If 20% of staff have not been vaccinated. To be able to take part in the scheme, the home will need to evidence that they have tried to increase the number of staff that have been immunised through communicating the benefits of the vaccination to them.

17. Indicator 1 - Falls Assessment & Care Plan • Improving falls assessments and falls prevention in care homes What is Required? Homes are required to: • complete a falls assessment of residents in the care home using the falls assessment and care plan supplied (C1). • The home must submit a copy of all completed falls assessment and care plans as evidence for this CQUIN Scheme.

18. Indicator 2 – Training • Improving falls awareness and prevention in care homes • Increasing staff training in falls and bone health awareness What is Required? Homes are required to: • encourage all members of staff to complete the free 30 minute training for care home staff on www.medicaleducation.co.uk • commit 2 senior members of staff and the Falls and Bone Health Champion to attend the falls awareness training held by Torbay Care Trust and South Devon Care Trust.

19. Indicator 3 – Calcium and Bisphosphonate Protocol • Improving bone health and falls prevention in care homes • Improve staff awareness of the importance of taking calcium and bisphosphonate at the right times. What is Required? Homes are required to: • sign and return a copy of the calcium and bisphosphonate protocol form to confirm that you have read it and will follow it and that all administering staff understand it and will adhere to it. (C3)

Indicator 4 – Falls and Bone Health champion • Improve staff knowledge and awareness of Falls and bone Health Homes are required to: • commit to a member of staff becoming a falls and bone health champion, understanding the level of commitment that is involved and the duties they will be expected to undertake. • provide a name, job title, address, contact number and email address of the nominated member of staff using the form supplied(C4). What is Required?

Indicator 5 – Strength and Balance Class What is Required? • Homes are required to run a twice weekly (1.25 hours) group for 6 weeks using evidence based exercises for strength and balance, either using • Evidence based Exercise programme booklets • The Age UK DVD (Be Strong, Be Steady) £12 plus p&p • postural stability instructor contact jane.reddaway@nhs.net or andy.simpson@nhs.net • An attendance register will need to be completed for each participant with brief notes about their ability to participate and individual progress through the 6 weeks.
