  2. Cervicalgia : - cervico-cefalea - cervicalgias y torticolis - cervico-omalgia y cervico -braquialgia Dorsalgia : localizada – irradiada ( intercostal ) Lumbalgia : alta – media – baja Lumbociatica Dolor sacro-iliaco Raquialgia global Raquialgias : según su localización

  3. Mecanicas Inflamatorias Infecciosas Metabolicas Malignas Vasculares Referidas o irradiadas Raquialgias : según su etiopatogenia

LUMBALGIA MECANICA • Sectorial baja, matinal, mejora durante el dia. • Exacerbada por extension, post-flexion, estacion sentada o de pie prolongada, pre-mestruo. • Mejoran con la deambulacion y calman con el reposo. • Relacionadas a causas de hiperfuncion lumbar. • Sin manifestaciones generales ni organo-funcionales. • Examen poco expresivo . Contracturas que desaparecen con el decubito; movilidad de raquis variable. • VES normal.

  5. SIGNOS DE ALARMA • DOLOR QUE AUMENTA CON LAS MANIOBRAS DE VALSALVA • LASÈGUE (+) • DEFICIT RADICULAR L4 / L5 /S1 • RETENCION DE ORINA / INCONTINENCIA FECAL / HIPOESTESIA DEL PERINÉ (CAUDA EQUINA )

  6. LUMBALGIAS INFLAMATORIASy SACRO-ILEITIS REUMATICAS (ESPONDILOARTRITIS) COLOPATIAS INFLAMATORIAS. PSORIASIS. REITER. ESPONDILO-ARTROPATIAS INDIFERENCIADAS . ESPONDILO - ARTRITIS ANQUILOPOYETICA.

ESPONDILOARTRITIS • Comienzan antes de los 45 años. • Antecedentes familiares / HLA B27 + • Aparicion insidiosa. • Permanentes ,intensas, a predominio nocturno. • Despiertan al pac. en la 2ª mitad de la noche. • Irradiacion ciatica bilateral alternada (en balanza) • Mejoran con el ejercicio pero no con el reposo.

  9. ENFERMEDADES ASOCIADAS A LAS ESPONDILOARTRITIS • DISENTERIA ( SALMONELLA, SHIGHELLA, CAMPILOBACTER ) • URETRITIS(CLAMIDIA , MYCOPLASMA ) • COLOPATIA INFLAMATORIA( CUC -CROHN) • PSORIASIS
