Services • Sick and well care • Women’s health • Travel Medicine • Detailed consultation regarding your trip • Specialized immunizations (e.g., Yellow Fever) • Sports medicine • Immunizations • Vaccines for Children (VFC) participant – provides vaccines for those <19 years old if uninsured for administration fee (currently $14) • Prescription Assistance • Will help students enroll in pharmaceutical company’s free prescription program if they meet financial requirements (not all medications have programs)

  4. Clinic Hours • Visits are by appointment ONLY • Typically can be worked in that day or next • Ill patients will be triaged by our nursing staff • Monday – Friday: 8 – 5 pm • Saturdays: 9 – Noon • except holiday weekends • Tuesday evenings until 7:30 pm during Fall and Spring semester • Closed all University holidays and closure days • After hours nurse line available

  5. Billing • Student Fees allow us to discount our services • We are happy to bill most insurances • If we are aware that a student’s private insurance does not reimburse us for our services, we will assist the student in finding a clinic that does (typically, a University clinic within our building). • The student is responsible for any charges not covered by their insurance. • Certain insurances (Kaiser) provide no coverage outside of their coverage area.

Student Insurance Plan2012-2013 • Waiting period for coverage of Pre-existing conditions are waived for participants under 19 years of age • $10 co-pay at Student Health Center • Wellness benefit (Annual Exam, and immunizations ) covered at 100% at Student Health Center • No annual deductable for services provided at Student Health Center • $100 annual deductible for services outside Student Health For complete details visit the website www.utesstudentinsurance.com

  7. Immunization Requirement • Two MMR’s (Measles, Mumps, and Rubella) are REQUIRED • Fill out the “Proof of Immunity” form and return to Student Health Center before September 30th • Only religious or medical exemptions are allowed

  8. “I can’t find the dates of my MMR” • For Utah students, the dates are usually available through your high school • If you are sure you have had the immunizations, blood titers can be drawn • However, if you are negative, you will then need the vaccination. • Insurance may cover the cost of vaccinations
