2007 Award Program • The Community Connections Award recognizes hospitals that have distinguished themselves through efforts to improve the health and well-being of their communities. • This award will annually recognize six hospital-based programs (one in each district of the state) that provide a significant community benefit.
Award Criteria • Leadership - Program takes a lead role in promoting and improving the health and well-being of the community. • Community Need – Program addresses a demonstrated community need. • Community Benefit – Program demonstrates a financial and/or resource investment from the hospital into the health and well-being of the community.
Award Criteria • Creativity - Program demonstrated a creative approach to providing a significant community benefit. • Impact - Program modifies behavior or educates the community. Program improves the health and well-being of the community (as a whole or to specific targeted populations).
Award Program • Hospital stories received from KHA members are considered by the KHA District Chairs and Vice Chairs. • Their recommendations are then presented to the Executive Committee of the KHA Board for approval. • One award will be granted in each hospital district, for a total of six awards annually. • There is no limit on the number of stories hospitals can enter.
Award Recognition • Awardees highlighted during KHA Spring District Meetings; • $1,000 to their community program; • Coverage in KHA publications and Web site;
Award Recognition • Recognition at a KHA-hosted media/award event in their community; • Statewide media announcements; • Community Connections Award.
