Short Term Disability Income Coverage Disability types: • Sickness disability • Non-occupational injury disability • Disability due to pregnancy (when disability begins 10 months after the effective date) • Disability due to complications of pregnancy Guaranteed renewable to age 72 There is no coverage for mental or nervous conditions
Total Disability/ First 12 Months • Total disability is where the insured is: • Unable to work at his or her SPECIFIC JOB;and • Not, in fact, working at his or her PLACE OF EMPLOYMENT for pay or benefits, and is • Under a doctor’s care for the covered injury or sickness causing such total disability • There are no benefits for partial disability • Benefits are payable for only one disability at a time
Portability • The employee can keep coverage regardless of employment or changing jobs as long as the premiums are paid and first deduction is taken • No change in premium rates • No change in benefits
Premium Rates • Unisex - same rates for males & females • Uni-tobacco - same rates for tobacco users and non-tobacco users • Rates are level and do not change as the employee gets older • Age Banded:17-49 50-59 60-67
Plan Design • Elimination Period: 0 Days for Accident 7 Days for Sickness • Benefit Period: 6 Months • Disability benefits are paid on the same frequency as payroll
Benefit & Underwriting • Up to 60% of gross salary may be covered • Up to $6,000 monthly benefit • Guaranteed issue up to $3,000 monthly benefit • Participation requirements may apply
