Building Relationships Conducive to Coordination Take Time • S&DS and Lipa (now Trillium) began meeting several years ago to work through issues/problems/concerns • Key Management staff in both organizations were committed to working together • As a result, lines of communication were opened and each organization had a greater understanding of each of our roles
Building a Relationship Conducive to Coordination Takes Education • Learning each other’s language • Understanding each other’s requirements • Understanding each other’s limitations • Understanding each other’s systems • Education of each other is ongoing
Building a Relationship Conducive to Coordination Takes Shared Experiences • Over time, S&DS and Trillium participated in several joint activities and projects • Real Choices Systems Change Grant • Lipa/S&DS Report on ED Admissions • Community Based Care Transitions Project • Planning for Health Systems Transformation • All of the above built the stage on which our current activities reside
