Coordinating Council on Access and Mobility (CCAM) • Executive order 2004 • Bring together Federal agencies • Simplify customer access to transportation, Reduce duplication of transportation services, Streamline federal rules and regulations that may impede the coordinated delivery of services, and improve the efficiency of services using existing resources. • Link to CCAM What could this look like at the State and local levels?

  5. Social Determinants of Health High-quality health care is essential for treatment of individual health conditions, but it is not the only tool at our disposal. In order to solve the fundamental challenges of population health, we must address the full range of factors that influence a person’s overall health and well-being. From education to safe environments, housing to transportation, economic development to access to healthy foods—the social determinants of health are the conditions in which people are born, live, work, and age Public Health 3.0 A Call to Action to Create a 21st Century Public Health Infrastructure
