May 8, 2006 a B1-B Lancer made a gear up landing at Diego Garcia skidding 7,500 ft down the runway. The aircraft was landing at the end of a 11 hour ferry flight from Guam During landing it caught flames and emergency crews had to extingust the flames The crew escaped and the investigation concluded the pilots forgot to put the gear down
The USAF claimed damage to the aircraft at 7.9 million dollars and $14,025 to the runway
Fatigue degrades every aspect of performanceresulting in : Slowed reaction time Impaired judgment Higher error rate Poor decision making ability Poor coordination
The two crew members on Corporate Airlines Flight 5966 were joking with each other before it crashed.They were discussing food and other coworkers. They ignored guidance about when and at whatspeed to descend the plane on its path to the runway, joking and cursing at one another while the plane's warning system alerted them of the rapidly approaching ground below
The pair were trying to land their sixth flight of the day after more than 14 hours on the job, Oct. 19, 2004, the crash was blamed on the crew's failure to follow proper procedures in preparation to land the plane 11 crew and 13 passengers died
Shift work puts you at a greater risk for fatigue Overtime gives better wages but puts a worker at high risk for fatigue
Things to fight fatigue Should eat properly Ask others to check your work Avoid complicated tasks when you are too tired Get the sleep you need
Stress Stress-- is any outside force or event that has an effect on our body or mind. Sky diving clubs first flight of the year The pilot survived
Stress can be caused by trouble at home Or at work Or at play
Pressure We feel pressure when we are asked to do something we feel we can’t do due to lack of resources, like time or tools Do we make our own pressure???
We need the aircraft by 6pm !!!!!! YES- you take on all the responsibility NO-insubordination, confrontation
Buy yourself sometime: Let me check the workload and I’ll get back to you Get the other person involved: We don’t have the manpower to get it ready by 6pm . Do you still want us to try even if it is late
Assertivness Do not let someone force you into a situation that you feel is unsafe
Lack of awareness Not taking into consideration all things when problem solving Putting a fire extingusher where someone will hit their head,is not thinking ahead
Lack of Resources When you don’t have the proper parts don’t feel pressured to make it work.
Lack of Knowledge Don’t work on something if you do not have the training Not having up to date manuals can be a huge problem
When all involved are not working toward a common goal it can be very frustrating But everyones job gets easier if everyone works together
Norms That’s OK it is always like that
Complacency Just because it was checked 100 times does not mean somthing may be found on the 101st.
Distraction Don’t allow outside forces to take your focus on the job
Lack of communication Hey take off the right engine He said right........ I’ll get the tools Did he say right or left??
