  1. Intelligent Clinical Training Systems Siriwan Suebnukarn DDS, PhD Advanced Dental Imaging and Computing Research Center Faculty of Dentistry, ThammasatUniversity ssiriwan@tu.ac.th

  2. Intelligent Clinical Training Systems Clinical Settings Patients Experts Intelligence Novices Equipments

  3. 1. System development With recent advantages in biomedical imaging and virtual reality technology, clinical training is entering a time of change towards more realistic and interactive environments.

  4. 1. System development Virtual Reality refers to a human–computer interface that facilitates highly interactive visualization and control of computer-generated three-dimensional scenes and their related components with sufficient detail and speed so as to evoke a sensorial experience similar to that of a real experience. Haptic systems include tactile information (force feedback).

  5. 1. System development Force Computation 1000 Hz Simulation and Update (shared data) Collision Detection 30 Hz Graphics Rendering

  6. 1. System development

  7. Process Assessment experts novices Tool path indicating the position of the tip of the instrument during tooth preparation of expert and novice. Blue area defines a region including 95% of the position samples.

  8. Process Assessment Hidden Markov Models novice logs TRAIN expert logs TRAIN

  9. Process Assessment New procedure TEST Log Likelihood (Novice) TEST Log Likelihood (Expert) Which one is higher? { Log Likelihood (Novice), Log Likelihood (Expert) }

  10. Tool Path Assessment To determine the potential of Dynamic Time Warping in matching novices’ cutting sequences with those of experts’ sequences.

  11. Outcome Assessment Outcome Measures of Expert/Novice Performance Support vector machine classifier

  12. Haptic Feedback The expert’s tool (E) movement is played back and a student (N) tries to learn the path by following the movement of the expert’s tool. The expert was pressing the tooth surface with force F, the force F′ with the same magnitude but opposite direction is rendered to the novice’s stylus. In order to keep up with the expert’s movement, the novice has to apply force F to cancel out the force F′.

  13. Surgical Skill Acquisition Evaluation The benefits of cone-beam computed tomography (CBCT) imaging have been in providing additional information for diagnosis and enabling more predictable management of complex endodontic problems (D’Addazio et al. 2011, Faitaroni et al. 2011). D’Addazio et al. 2011

  14. Surgical Skill Acquisition Evaluation With the advent of modern endodontic surgical concepts, failed RCT teeth can be preserved utilizing endodontic microsurgery that combines the magnification provided by the microscope with the use of microinstruments (Kim & Kratchman 2006). Kim & Kratchman 2006

  15. Surgical Skill Acquisition Evaluation Endodontic surgery is perceived as difficult because the surgeon must often approximate the location of anatomical structures, clean and fill all the complex canal apical ramifications. To design and evaluate the impact of virtual reality (VR) pre-surgical practice on the performance of actual endodontic microsurgery (Suebnukarn et al., Int Endod J 2012).

  16. Surgical Skill Acquisition Evaluation A significant difference was observed between the scores for osteotomy on a molar tooth completed with virtual pre-surgical practice and those completed without virtual pre-surgical practice, median 4.5 (range = 3.5–4.5) versus median 3 (range = 2–4), P = 0.042.

  17. Discussions The intelligent VR dental training system could be an invaluable tool for training in developing nations where there may be a dearth of teaching dental practitioners and may point the way in the development of VR training tools for other kinds of medical procedures.
