OUR VISION: Small Scale Changes driving Large Scale Improvement Supporting self-management of nutritional care for people with LTC Improving meal processes: improving intake/reducing waste Preventing re-admission due to malnutrition Managing transitions between hospital and care home to avoid malnutrition Ensuring people get the support they needto eat and drink
Testing NHS GGC • Background • Right Patient Right Meal Right Time (2008) • Compliance with CQI measurements • Better Together results • Aim • To undertake an observational audit of patient mealtimes • Methodology • Crib sheet developed to increase continuity • Briefing session for dietetic students • Observed 3 consecutive meals in 1 ward
Testing NHS GGC • Results • Mostly prepared for mealtimes • All patients who required assistance received it • Staff were available but not all engaged • Different experiences on same ward dependant on staff • AHP’s on wards speaking to nursing staff • Patients interrupted for procedures by doctors at mealtimes
