2. History 52 Years old male Worker in a tyre factory, chronic smoker H/o type 2 diabetes mellitus and systemic hypertension -one year. Coronary artery disease - anterior wall myocardial infarction in 2007 Was on alternative medications Admitted with nausea, vomiting, breathlessness and decrease in urine output.
