How is reablement delivered? • A short-stay accommodation based service – up to 6 weeks • Designated self-contained furnished flats • Based in a 24/7 staffed Extra Care setting • Up to 20 hours of 1-1 support • Delivered by a multi disciplinary team • Directly accessed from the hospital or community • Assessments on the ward • Intensive, time limited and goal orientated Subtitle
