Tranformative Leadership: The Experience of a First World Country/ Developing Country Collaboration Dr Yvonne Osborne y.osborne@qut.edu.au
Impetus for Change • Driven by health care reform and the advent of the private sector in the late 1980s, and by commitments made to the Association of Southeast Asian Nations (ASEAN), Vietnam is faced with a need to increase the regulation and training of its health care professionals. Previously, a diploma from an accredited health professional school was sufficient to practice for a lifetime. Legislation has recently been passed that will institute a licensing system, will require continuing medical education (CME) to maintain the license, and will probably place a large burden on the health professional schools and training institutes to provide CME. Supported by international nongovernmental organizations and foreign universities, the 12 medical universities in Vietnam are responding and are preparing for their new and expanded role. (2006)
The QUT-AP Project 2008-2012Building Capacity for Nurse Education in Viet Nam • WPSEAR Conference • Forming partnerships with 2 lead universities and 5 College partners • Scoping study QUT Legislation for licensing of health professionals • Signing of Mutual Recognition Agreement with ASEAN Nations • MOH adopts Competency Standards for Bachelor Nurses MOET accepts graduate nurses need to be competent
Transformation • Biomedical model taught by medical doctors resulted in nurses as assistants to doctors and disease focussed care • Government highlights that standards for nursing education do not meet international requirements • Move toward person centred, holistic care that focussed on nursing knowledge • Change in teaching methods to develop evidence based decision making and critical thinking
Transaction • Partnerships formed with lead universities • Hanoi Medical University (partnered with Hanoi Medical College and Hai Duong Medical Technical University • Ho Chi Minh City University of Medicine and Pharmacy (partnered with Khanh Hoa Medical College, Binh Duong Medical College and Can Hoa Medical College) • Vietnamese Nursing Association • Contracts negotiated and signed (goals and resource support agreed to)
Transformational Leadership • Taking the lead and helping followers (nurse lecturers and nurses) to understand the model of competency based education through workshops(n=300), publications, critical challenges, communication (skype and video conferencing), distance education (n=180) • Establish agreed to goals for development and agreement on how to work toward those goals (these reflect MOH and MOET requirements)
Participative Leadership • Key nurse teachers and nurses start to work with expert to progress the curriculum work • Problem: context both locally and nationally is not supportive to such changes • Sharing ideas and strategies for overcoming barriers
Servant Leadership Model • The expert now understands the followers and the context within which they practice. She can see clearly now! • She has taken a back seat but is available when needed • The nurse teachers and nurses who have participated in this project are ‘doing it for themselves!’
It is better to lead from behind and put others first, especially when you celebrate victory when nice things occur. You take the front line when there is danger. Then people will appreciate your leadership. (Nelson Mandela)
Recognition of success • Adoption of Competency Standards for Bachelor Nurses (VNA) • Adoption of CBC for all nursing education in Viet Nam by MOH • Formation of Nurse Teacher Branch (VNA) • MOET agrees that nursing graduates must be competent to practice • 2 honorary professorships awarded by lead universities • A country medal presented by the Government for work in nursing education • Consultation in accreditation of courses for health professionals (MOH) • Consulting with medicine on developing their CBC • For me however: • Nursing is now going to be taught by nurses to a standard that is equivalent to global standards set by WHO, with improvement in nurse:doctor ratio, and increasing applications by nurses for sholarships to post graduate studies
Finally • A leader is best when people barely know he exists, when his work is done, his aim fulfilled, they will say: we did it ourselves. (Lao Tzu)
