Qualifications • Company Grade Officer and enlisted (Brooks City-Base awards) • Be on extended active duty and assigned to a Brooks City-Base (BCB) organization (includes tenant units) during the award period. For annual award submission, nominee must have been assigned to BCB for a minimum of six months for enlisted and eight months in current job position along with one year in AFMC for CGOs or (enlisted only) be a previous base-level quarterly award winner. Tenant units are eligible to compete for the BCB Annual Award, however submission to the Aeronautical Systems Center Annual Awards will be the top nominee from within the AFMC chain of command.
Qualifications • Be serving in a grade appropriate for the award on the last day of the award period for quarterly nominations. For enlisted, annual award nominees compete in the category of the grade held for the longest period of time during the award period. Officers must be CGOs on 31 Dec to compete for annual awards. • Not have been punished by court-martial or Article 15 during the one-year period immediately preceding the award period. • Not have a UIF or have been placed on Control Roster during the award period.
Qualifications • Civilians (Brooks City-Base awards) • Be assigned to a BCB organization during the award period. For annual award submission nominee must be assigned to BCB for a minimum of six months. • Be serving in a grade appropriate for the award on the last day of the award period. For annual awards, nominees compete in the category of the grade held for the longest period of time during the award period. • Not have received an unfavorable personnel action during the award period.

Company Grade Officer (CGO) • CGO Quarterly Awards (20 lines) • LEADERSHIP AND JOB PERFORMANCE IN PRIMARY DUTY (10 lines, max 30 pts) • Rate the members’ leadership and job performance in their primary assigned duty, including consistency and creativity in duty performance. Points are awarded based on the degree to which the nominee’s performance contributed significantly to increase mission effectiveness during the current calendar quarter.
Company Grade Officer (CGO) • CGO Quarterly Awards • SIGNIFICANT SELF-IMPROVEMENT (5 lines, max 15 pts) • The member must have shown personal efforts towards self-improvement through off-duty education, achievements in professional or cultural societies or associations, development of creative abilities, etc. Assign points based on the degree to which the nominee has been formally recognized for his/her accomplishments.
Company Grade Officer (CGO) • CGO Quarterly Awards • BASE OR COMMUNITY INVOLVEMENT (5 lines, max 15 pts) • The results of the nominee’s accomplishments must set them apart from others of equal or higher grade through the display of leadership qualities and attributes, both on and off duty. Demonstrated ability as a positive representative of the Air Force, to include participation in base and local community activities and programs.

Include any off-duty education not directly related to primary duties; e.g., class, course, degree enrollment and/or completion, grade point average. Cite any other relevant training or activity that significantly enhanced the member’s value as a military citizen.

Air Force Medical Service • Team (one page) • Team Goals (10 pts): the team’s annual goal(s) that support AFMS and local strategic goals with related outcome measures. • Team Accomplishments (40 pts): list actual or expected outcomes. When outcomes have not yet been realized, describe the expected/intended outcomes. If expected outcomes were not achieved, team efforts to redefine the goal/outcome should be included. • Community and Customer Service (20 pts): efforts employed by the team to identify and meet community and customer needs and desires. • Team Performance (20 pts): Other team accomplishments that demonstrate integration, collegiality, and significant contribution to increased mission effectiveness. • Outstanding Team Members (10 pts): significant self-improvement of individual team members (e.g., off-duty education) or contribution by a team member (s) to professional societies or associations.
