  6. 4 Essential Components • Details of the cancer diagnosis and treatment information (Treatment Summary) • Overview of physical and psychosocial effects of diagnosis and treatment • Detailed plan that outlines surveillance for early dx of recurrence or secondary cancers and late effects of treatment as well as recommendations for health promotion strategies • Referrals and resources for physical, psychosocial and practical needs.

  7. SCP Templates • Several available • Paper • Web based • Organization developed and supported • Institutional Advice for whatever SCP path is chosen that Institute of Medicine’s recommendation for content is followed.

Journey Forward • Tool created by the collaborative efforts of: National Coalition for Cancer Survivorship The UCLA Cancer Survivorship Center WellPoint Inc Genentech --- Has the 4 essential components outlined by IOM --- Free online program www.journeyforward.org --- Can be downloaded to desktop and uses Cnext Info where available
