Mrs J • Mrs J is an 85 year old lady who has history of: • COPD, Non insulin dependant diabetic, Previous MI ,and Angina • She has had several (more than 3) admissions over the last year for exacerbation of COPD, she is becoming increasing frail, and has symptoms of breathlessness and anxiety. She is on home oxygen. She lives with her elderly husband who is fit and well.
Mr G • Mr G is an 84 year old man who has dementia. He has been admitted from a nursing home with confusion, he has a raised temperature, he is dehydrated and has ‘gone off his legs’ ? Due to chest infection or UTI. He has recently been losing weight, unable to walk or dress without assistance, and is becoming increasingly frail.
Mrs C • Mrs C is a 94 year old lady who has a history of Parkinson's disease, Chronic Cardiac Failure, and osteoarthritis. She has been admitted from home ‘off legs’, and confused (UTI). She has been deteriorating over the last few months becoming increasingly frail, losing weight, having swallowing problems, and less interested in things. She lives with her younger sister who is 80. Despite treatment with antibiotics her condition continues to deteriorate, she is now sleeping most of the day, her PPC is home.
