It is the mission of TRACE to prepare students (with support from staff, friends, and family) to make informed decisions regarding present and future plans about vocational placement, living arrangements and recreation. ALL STUDENTS BELONG TOGETHER IN THEIR SCHOOLS AND COMMUNITIES. ALL MEANS ALL
Individuals with Disability ActIDEA • Section 300.29Transition Services (1) Is designed within an outcome-oriented process, that promotes movement from school to postschool activities, including postsecondary education, vocational training, integrated employment (including supported employment), continuing and adult education, adult services, independent living, or community participation.
The Person Centered IEP/ITP Process is a five step method to help educators identify student needs. This method will help determine IEP/ITP goals, which will be based on Person Centered Based Student Assessment. 1 Transition Domains Assessment - Vocational - Recreation/Leisure - Self-Advocacy - Independent Living - Community - Adult Education Person Centered Based Student Assessment 4 PCP Develop Data Sheets and Data Collection Methods for all IEP/TP Goals IEP Goal Progress Reported, TCM Filemaker Schedules: Staff, Student and Site SOP for students in Final Year Anecdotal Observation Assessments as appropriate i.e. 3 year, SLP, Behavior Support 2 Ongoing Instructional Decision Making including DIS Services 5 Analyze and Summarize Assessment Information into Present Levels of Performance Report Access to Teacher Resource Center, Sharepoint and Help Camps Link Curriculum & Instruction with IEP and Transition Plan Goals and Assessment Scope & Sequence Quality Indicators & Staff Training Logs Summarize strengths, interests, learning preferences and progress on previous IEP goals Demonstration Lessons and Materials Modifications & Accommodations • Identify IEP/TP needs based on Person Centered Based Student Assessment • Academic/Readiness • Communication • Motor skills/Sensory Motor • Social/Emotional/Behavior • Pre-Vocational/Vocational • Self-Help • Health • Identify Transition needs based on Transition Plan 3 Develop IEP and Transition Service Needs Person Centered IEP/Transition Plan Process
Who comes to TRACE? • Must be 18-22 years old • Special Education Students who are eligible for an IEP • Have not received a High School Diploma • Must live within SDUSD boundaries
Where services are provided: • Students receive services in their own community • Support is provided based on student needs
