What is the School Based Clinic? It’s a clinic, in your school! We provide high quality, affordable and comprehensive medical and behavioral health services by professional staff specializing in adolescent health. • Our services are confidential – we won’t tell anyone, not even your parents, when or why you came into the clinic. • Convenient – we are located right in the school! No need to worry about transportation or missing lots of class time. • Affordable – everyone is covered, regardless of insurance or ability to pay. If you have insurance, we will bill it, but you will never have any out-of-pocket costs.

Our Services • Physical exams/checkups – for sport, college, jobs, etc. • Immunizations • Mental health, social work • Nutrition counseling • Birth control & pregnancy testing • Sexually Transmitted Infection testing & treatment • Help getting health insurance
Other programs: Teen Council Citywide teen health council, students from all 7 high schools with a Minneapolis school based clinic • Advise the school based clinics • Talk about health topics with your peers • Develop health outreach projects • Gain skills: leadership, teamwork, communication, advocacy, marketing… • Service hours • Fun! Registration & Consent Yellow registration & consent form Must be completed and signed by your parent/guardian if you are under 18 and returned to the clinic – once we have this form, it is good for all 4 years! Minor’s Consent Minor’s consent is an exception to the rule that your parents have to agree before you can receive health services if you are under 18. It only applies to some services though. At the clinic, this means you can complete your own pink form and be eligible to receive certain services such as those related to reproductive health (like birth control, condoms, pregnancy testing) and drug/alcohol abuse. Visiting the Clinic – It’s Easy! Step 1: Register to use the clinic Step 2: Stop by the clinic, Room 107, and make an appointment with Dawn at the front desk. She will help you pick a day and time that fits with your schedule. Step 3: You will get a card with the date and time of your appointment. Hold on to this, or write it down somewhere else so you don’t forget! Step 4: On the day of your appointment, your teacher will get a pass from us that excuses you from class. Come on down and check in with Dawn. Step 5: Your provider (Lori, Sharron, Mary) will greet you and you can get started with your appointment! If it is your first time to the clinic, you will have to fill out a health history form. Step 6: Check out with Dawn and schedule a follow-up appointment, if needed.
