WHO WE ARE: • We are an urban school district in the heart of Phoenix in an area known as Maryvale. • We service over 19,000 students daily • 20 Schools consisting of: • 4 = k-8 • 4 = middle schools • 12 = elementary • Median income per family: $35,000(per family, Maricopa County) • Percentage of students on free or reduced lunch: • 81.55% (September 2012) • Home values: $33,260 (October 2012, Maricopa County Elections) • Number of foreclosures: 1,315 (September 2012, City of Phoenix)

WHY? • Proximity/convenience to members of our community. • We are very pleased to host the clinic 3 days a week (Monday, Tuesday & Thursday 8:00 a.m. – 4:30 p.m.). • Keep our clinic busy: make our parents aware, train our staff to ensure the clinic stays busy. • 1,117 students medical needs served since inception (September 2009). • Reduce school absenteeism & improve academic achievement: • Over 80% of children seen, return to class allowing them to focus on learning. • Reduction in absenteeism: yes (5.1%) (district data) • Retain & increase student enrollment.

  4. HOW? • Propose to Executive Team • ABRAZO draws up School-based Clinic Agreement • Provides Certificate of Liability Insurance • Attorney review • Find a centralized space that provides a separate & secure entrance. • Coordinate with ABRAZO to ensure space is sufficient for their needs. • Obtain Governing Board approval • Allow ABRAZO staff access to facility.

  5. GOVERNING BOARD ENGAGEMENT: • Explain why it is critical to underserved communities. • Access • Awareness • Provide primary medical care, management of chronic illnesses, and some dental services. • Keep your Governing Board informed about the clinic: • Provide statistics • Refresh their memories about the services provided • Remind them why it is important to provide this service to our community.
