Agenda • Challenges in Service Plan Development • Incorporating Multiple Data Sources into the Service Plan • Helping Families Utilize the CANS Tool for Service Plan Development.
Challenges in SERVICE PLAN Writing • When do you experience Service Plan Writer’s Block? • What are common challenges preparing and writing the Service Plan?
Challenges in SERVICE PLAN Writing • Word Usage • Reframing • Narrowing down the goal and objectives • Keeping it simple, attainable, measureable • Specific vs. General (real life vs. lab) • Incorporating multiple data and perspective sources.
Multiple data sources • Who is involved in Service Plan development? • Motivation and Perspectives can be different depending on the source.
Multiple data sources • What happens when there are many perspectives on what the goals/objectives should be? • Person-Centered Approach. • Child “Readiness”. • Clarify Waiver mission, scope, and boundaries. • Be creative in connecting differences (sometimes the goal is the same but the approach looks different). What do you see?
Multiple data sources • How can we utilize ALL players to address the many perspectives in the Service Plan? • Teamwork • Brokering • Connect the Roles • Validate Motive • Part of Success • Accountability
Multiple data sources • Many different sources of input • can influence the overall completion • of the CANS. • Variables that influence perspective • Be objective and use “evidence”
Making Sense of the CANS • We use 2s and 3s as areas of • focus in the development of the Service Plan. • Making 2s and 3s meaningful for the • family (recognizing the need, prioritizing) • Operationalizing 2s and 3s into objectives • (turning deficits into strength-based objectives….reframing)
Motivational Service Planning • We may see areas of need/strength • not noted by the family. • Motivational service planning: Can we • support areas of need directly AND indirectly? • Motivational service planning refers to the • child/family’s readiness for: MOTIVATIONAL SERVICE PLANNING
Reframing • It may be difficult for a family to reframe experiences, challenges, and concerns in a positive, goal-oriented fashion. • How do we turn “can’t” and “don’t” into “can” and “will”? • Example: Kendra won’t hit her brother when she becomes angry. (Let’s reframe this statement.)
Service Planning Scenario Writing a Goal that Addresses many Needs/Concerns. Purpose of Skit: To demonstrate some of the difficulties we may encounter in combining the CANS Scores, the Child’s priorities and the influences of Others into the Service Plan.
Skit (BACKGROUND) • Client- “Johnny”: 17-year senior student hoping to go to university in California • MH Diagnosis: ADHD, ODD • Father left home/family with no goodbye 3 weeks ago • 7-day psychiatric hospitalization: suicide attempt, “took a handful of” Prevacid • (thinking it was mother’s Xanax) • Referred to case management as part of psychiatric (hospital) discharge plan (CANS provided by treating psychiatrist)
Overcoming SERVICE PLAN writing challenges • Goal: I will complete thorough and strength-based service plans. • Objective: I will listen to the concerns and strengths of the child/family during each visit • Objective: I will demonstrate during each visit how to transform concerns and “deficits” into goal-oriented statements. • Objective: I will incorporate multiple perspectives regarding the families progress while maintaining a client-centered service. • Method: Who helps us when we struggle with SP writing? Is this an area where I can use the other systems/supports? • Who are those supports? • What resources exist that help with SP writing?
