HEALTH ECONOMICS, MPH SPRING 2009 Costs and effects of introducing a schoolbased intervention on one hour mandatory physical activity per day Solveig Forberg Axelsen, Eva Birke, Hanne Borch-Christensen and Gitte Frydenlund
BENEFITS OF PHYSICAL ACTIVITY • Develop healthy musculosceletal tissues • Develop a healthy cardiovascular system • Develop neuromuscular awareness (coordination and movement control) • Maintain a healthy body weight • Psycological benefits (control over anxiety and depression) (WHO)
FOUNDATIONS/BACKGROUND • WHO estimates that physical inactivity results in 1.9 million deaths yearly world wide • 60% of adults worldwide are non-compliant to recommendations • Social inequality in physical activity (poor people have less free time and poorer access to leisure facilities)
PHYSICAL INACTIVITY IN DENMARK • 7-8 % of all deaths (4.500 persons/year) • 50.000 lost life years equally distributed among men and women • Accounts for a reduction in life expectancy of the Danish population of 9-10 months • Physical inactivity reduces QALY’s by seven years. • Accounts for 100.000 admissions to hospitals • 2.6 million contacts to GP’s per year • 3.6 million days of sickness absence from work • 1.200 health related early retirement pensions a year Risk factors and public Health in Denmark (2007)
INTERVENTION • 1 hour mandatory physical activity from 4th to 9th grade in all schools in DK • Purpose: improving long lasting habits with regards to physical activity in adulthood • Leading to a reduction in lifestyle diseases such as diabetes and coronary heart disease and a gain in QALY´s and life expectancy
COST EFFECTIVENESS ANALYSIS • Costs in DKK • Effect in QALY’s • Cost-effectiveness ratio
MODEL Decision tree A (20%) Intervention:1 hour physical activity per day B (40%) C (60%) No intervention
CONCLUSION • One hour mandatory physical activity per day in schools is a cost-effective intervention
