Volume License Claim Form: Part B • By checking the box in Part B, you are requesting that the Claims Administrator search Microsoft’s records for the State of Iowa’s volume license purchases. • You will receive a report listing all of the State of Iowa’s volume license purchases of eligible Microsoft products from July 1, 2002 through and including June 30, 2006 according to Microsoft’s records.
Volume License Claim Form: Part B • You will have 90 days to make corrections and/or additions to the State of Iowa’s report (including adding claims for purchases made outside the State of Iowa’s volume license agreements). • The State of Iowa’s claim will be automatically approved for any eligible volume license purchases that appear in Microsoft’s records.
