Achtergrond • Veiligheid bloedproducten zeer goed anno 2010 • HBV 1:220.000 • HCV 1:1.000.000-1:2.000.000 • HIV 1:2.000.000-1:3.000.000 • Bacteriologische contaminatie van m.n. trombocytenconcentraten blijft zorgenkind • 1:2.000-3.000 gecontamineerd • 1:20.000-1:100.000 septische transfusiereactie • Ness Transfusion 2001

  3. Achtergrond • Methoden om risico bacteriologische contaminatie te verminderen: • Desinfectie aanprikplaats • 10-15 mL via sample pouch • Reductie 0.85% tot 0.37% initieel positieve kweekuitslagen (56% reductie) • de Korte et.al Transfusion,2006

  4. Achtergrond • Methoden om risico bacteriologische contaminatie te verminderen: • Bacteriologische screening van trombocytenconcentraten. • Sinds 2001 in Nederland BacT/ALERT • Aeroob en anaeroob • Negative to date!

  5. Achtergrond • Alle trombocytenconcentraten worden ‘negative to date” uitgegeven • Gedeelte van reeds toegediende concentraten zal alsnog positieve BacT/ALERT krijgen • Vraag: • Treden er vaker transfusiereactie op bij tromboconcentraten waarbij alsnog BacT/ALERT positief werd?

  6. Methoden • Buffycoat-trombocytenconcentraat (5 donors) • 7.5 ml anaeroob en aeroob • 18-24 uur na volbloedafname • < 2 uur productie trombocytenconcentraat • Aferesetrombocyten • 7.5 ml anaeroob en aeroob • < 12 uur na afname

Methoden • Alle positieve flesjes naar bacteriologische lab voor confirmatie en determinatie. • Alle producten uit voorraad of recall uit ziekenhuis. • Indien product reeds toegediend dan actieve navraag naar transfusiereacties. • Alleen die producten waarbij positieve BacT/ALERT werd bevestigd (confirmed positives) in analyse opgenomen.

  9. Resultaten • N=160 positieve kweken • N=21 positief signaal aerobe flesje • N=139 positief signaal anaerobe flesje

  10. Resultaten • Diphteroiden: • Proprionibacterium species, Corynebacterium species, ongespecificeerde diphteroide staafjes, Gram+ staafjes • CNS+: • coagulase negatieve Staphylococcen subspecies, Gram+ coccen • Gram- staafjes • Bacillus species • Peptostreptococcen

  13. Resultaten • Klinische gegevens • N=158 patienten gegevens beschikbaar • Bij 2 patienten transfusiereactie • Beiden Propionibacterie gekweekt • Een patient allergische reactie met urticaria. • Andere patient koorts bij langer bestaande sepsis. • Beiden imputabiliteit onwaarschijnlijk.

Conclusie • Door screening BacT/ALERT wordt 2/3 van gecontamineerde trombocytenconcentraten niet getransfundeerd • Determinatie van positieve BacT/ALERT van eenheden reeds getransfundeerd vnl. niet pathogene huidbacteriën • Twee van de 158 trombocyten transfusies was geassocieerd met transfusiereactie met lage imputabiliteit

Bacteriologische surveillance van trombocytenconcentraten zinvol?Ja, maar……. • 184 tromboconcentraten met pos BacT/ALERT en pos. kweek geen transfusiereactie • Echter 2 patienten met septische transfusiereactie door Bacillus cereus met vals-negatieve BacT/ALERT • P. te Boekhorst • Transfusion 2005
