Introduction • Healing Pathway Analysis complements traditional value stream analysis and improves the patient’s experience • During the Healing Pathway Analysis • New future state was designed • Rapid improvement plan developed • Identified the actions required to achieve it • Challenges • Team members leave the event and go back to their day-to-day job. • Difficult for the leader and the team to stayed engaged, follow up, and bring the future state to reality. • “Checking the results and Acting to lock them in” ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Measurement • Healing Pathway results can be measured in two ways • Deployment • Outcome • Deployment – refers to the execution of the rapid improvement plan • For example: are the Just Do It events, rapid improvement plans, and projects being executed as scheduled? Measurement Cont. • Outcome – refers to the impact on key metrics of the healing pathway • For example: Does the execution of the rapid improvement plan yield the planned results? ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Leadership Role Objectives • Demonstrate by their participation and leadership that transformation of the healing pathway is a priority • Monitor healing pathway transformation efforts and sustain results • Identify and remove barriers • Continue to reenergize the transformation • Reprioritize efforts and initiatives as appropriate • Standardize Best practices ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
What Do I Do? • Develop a healing pathway dashboard • Meet biweekly to ensure execution • Review the dashboard • Review the rapid improvement plan • Eliminate barriers • Reprioritize as appropriate • Standardize Best Practices ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Pathway Dashboard • The tool the executive sponsor and transformation team should use to evaluate the outcomes of the healing pathway transformation initiative. • Should be updated weekly • Should Include: • Metrics that reflect the overall health of the healing pathway • The specific metrics associated with transformation efforts ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Pathway Dashboard Example • Dashboard for the Healing University ED project • Red: Inadequate or insufficient progress • Yellow: Improving but not at goal future state • Green: Future state performance achieved • LOS: Length of Stay • LWBS: Length Without Being Seen ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Biweekly Meetings to ensure execution • This step is one of the most critical for ensuring sustainment of the healing pathway transformation effort. • Each meeting should follow a standardized agenda ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Standardized Agenda • Executive sponsor’s welcome and remarks • Review of charter and key objectives of the healing pathway transformation • Review of actions from prior healing pathway transformation meetings • Review of healing pathway dashboard • Review of rapid improvement plan • Out-briefs by team leaders • Problem solving and elimination of barriers • Evaluation and reprioritization of initiatives as appropriate • Action item review ISE 428 ETM 528 ASB Lean Healthcare Chap: 14
Standardize Best Practices • Standardizing Best Practices at the healing pathway level means: • Implementing the desired future state (Best Practice!) • Developing and documenting new and revised procedures as appropriate • Ensuring these new procedures are fully implemented • Coupled with the biweekly transformation team meetings, this is the Check-Act portion of the healing pathway transformation.
