What to expect in 2019 • April – Official Launch • “Health Assessment” – (10pts) • Questionnaire (10-15 min to fill out) • “Blood Pressure Companion” – (5pts) • 30 Days • Start – April/May • If you want to track your blood pressure via phone app for this program try out “Blood Pressure Companion”. • Points throughout the program will earn you chances to will prizes and give-aways at the end of the year. The more you participate the more chances you have to win.
2019 Continued • May (End of) - Blood Work – (10pts) • Full blood panel (on-site), or at a local LabCorp facility • You can also turn in blood work from your family physician if it has been done between Jan 1st and July 1st 2019 • June (End of) - Reporting of results to your group (Blood Work/Health Assessment) • One Stop Wellness reports in Aggregate data ONLY to your employer. Your Health Assessment report will contain certain numbers from your Blood Work. We’ll compare these numbers for up to 5 years so you can see the trend of your health.
2019 Continued • June (End of) through early August - Lots of people out on vacations, etc. • *We try to minimize programming at this time. • August (End of) – “Pick Your Plan & Stick With It“ (5pts) • 30 days • October – Declutter to Destress (5pts) • 30 days
2019 Continued • November – Advertise total points earned. Don’t worry, we’ll report periodically how many points you’ve earned. We’ll also start talking about what’s to come in 2020. • *There are other items that you can complete that will earn points. So, stay tuned! • Race Event • Gym attendance – Report (provide average of 3 days per week over 8 weeks • Soccer league, hockey league, etc. • Webinar’s and Lunch-n-learns • All programming for this location must be approved by Jeffrey O’Brien/Gail Gleason
