AME MUST REVIEW ACCEPTABLE CERTIFICATION CRITERIA Treating physician finds the [ ] Yes condition stable on current regimen and no changes recommended Symptoms [ ] None or mild to moderate symptoms with no significant limitations to range of motion, lifestyle, or activities Cause of Arthritis Acceptable causes are limited to: [ ] Rheumatoid (limited to joint), psoriatic, or osteoarthritis Acceptable Medications [ ] One or more of the following: Oral steroid which does not exceed equivalent of prednisone 20 mg/day (see steroid conversion calculator), Methotrexate, hydroxychloroquine (Plaquenil - see mandatory eye evaluation requirement below), NSAIDS. Complete blood count (CBC) and [ ] Within 90 days complete metabolic panel FAA Report of Eye Evaluation Form 8500-7 is required if hydroxychloroquine (Plaquenil) is used. [ ] Normal CBC, Liver Function Test, and Creatinine [ ] 8500-7 Favorable and no concerns [ ] N/A AME MUST NOTE in Block 60 one of the following: [ ] Airman meets certification criteria for arthritis. [ ] Airman had a previous Special Issuance for this condition and now meets the regular issuance certification criteria for arthritis. [ ] Airman does NOT meet certification criteria for arthritis. I have deferred this exam. (Mail the supporting documents to FAA identifying which criteria were not met.)
Asthma • Your symptoms cannot result in using your “rescue inhaler” more than 2 X/week • You cannot have taken oral steroids for an acute attack more than 2 X/year • In the past year • No in-patient admissions • No greater than 2 O.P. Clinic/Urgent Care Visits for exacerbations • Can be taking any of the standard asthma meds
Asthma (cont'd) • Need Current Pulmonary Function test (not required if treatment is as needed use of inhaler 1 – 2 x per week)
Glaucoma • Condition is stable • 40 years or > when diagnosed • Unacceptable are conditions: • Normal Tension Glaucoma • Secondary Glaucoma from inflammation, trauma, or presence of other significant eye pathology • Most medications acceptable • Unacceptable: Pilocarpine, Atropine or oral meds
Glaucoma (cont'd) • No side effects of medication taking • Pressure in eye must be less than 23 • No visual field defects
Hepatitis C • Condition must be stable • No complications or symptoms • On NO medications to treat • Need Liver Function Tests within prior 90 days • NOTE: these policies are for your AME to issue without a special issuance but you may be able to get a waiver, just not be cleared by the AME
High Blood Pressure • Your AME could have always cleared you with this condition as long as you brought in the proper medical information! • Stable for at least 2 weeks • BP in AME's office no greater than 155/95 • Can be on up to 3 different medications • Unacceptable are medications that cause sedation = Clonidine (=Catapres)
Hypothyroidism • Condition stable • No symptoms of fatigue, mental status impairment, or pulmonary, cardiac or visual issues. • Acceptable medications: Synthroid, Levothyroid, Armour thyroid, Cytomel, and Thyrolar • Normal TSH level performed within 90 days.
Migraine & Chronic Headaches • Condition stable with no changes recommended • Acceptable type H.A's: Classic/Common Migraine, Chronic Tension, Cluster headache • In past year: • No in-patient hospitalizations • No more than 2 O.P. Clinic/Urgent Care visits for exacerbations • No neurological or TIA-type symptoms, Vertigo, Syncope, and or mental status changes
Migraine & Chronic Headaches (cont'd) • Preventive Medications: None or daily calcium channel or beta blockers • Abortive Medication Restrictions: • Triptans – 24 hour No Fly • Metoclopramide (Reglan) – 36 hour No Fly • Promethazine (Phenergan) – 96 hr No Fly
Pre-Diabetes Mellitus • Condition stable • No diabetic symptoms • No Low Blood sugar events or Glucose less than 70 within past 12 months • Fasting Sugar <126 • Current HgA1C level within past 90 days → < 6.5 • No medications or Only Metformin
Renal Cancer • Condition stable • No metastatic disease or evidence on Path report of extension of tumor beyond capsule • Treatment completed and back to full unrestricted activities
Future CACI's • Kidney Stones • Carotid Artery Stenosis • Colitis • Irritable Bowel Syndrome • Colon cancer • Bladder cancer • Leukemia • Hodgkin's disease and Lymphoma
