  2. Health Insurance: All students in New York City public schools up to the age of 18 are eligible for public health insurance at minimal or no cost. School records show that over 10% of our school children are uninsured (approximately 144,000). The Office of School Health collaborates with the Office of Citywide Health Insurance Access (OCHIA) to provide access to public health insurance to our students and their families through the schools.   3. Health insurance information of all children registered in your school must be entered into ATS. Codes: Covered: M: Medicaid (Child Health Plus “A”) B: Child Health Plus “B” P: Private Health Insurance (e.g. GHI, Blue Cross, etc.) Not Covered: S: DOE is allowed to have a health insurance enroller contact the family N: DOE is NOT allowed to have a health insurance enroller contact the family No data: C: The child’s record has not been updated in the last year U: Unknown Health Insurance (Cont.)

School-Based Health Centers (SBHC) • If your charter school is located in a building where there is a SBHC and your students are not enrolled in the SBHC, please contact our office. • Requirements to have a SBHC in your building: • Facility: 1,000 Sq.Ft (exam rooms, waiting area, lab, restroom, counseling room/s.) • Minimum of 1,200 students in the building. Parents must sign consent form for students to receive services.
