IMMEDIATE CAUSE • Poor holding ground (soft mud) and strong underwater current (Dragging of anchor is a common occurrence in this area). • Bridge Team failure - Inadequate bridge watch maintained at anchorage. • Non-reporting of anchor dragging to the Master which lead to the vessel movement not controlled. • Inadequate use of radar and vessels turning circle to determine dragging of vessel. • Specify any breach of company or legislative requirements identified: • 1. OMD 001 – Bridge team • 2. OMD 003 – Bridge watch
UNDERLYING BASIC CAUSES • Pilot advice not considered at the time of anchorage. Pilot advised master to ensure engine movement and steering to be used effectively to ensure vessel yawing is controlled. Excessive yawing could lead to anchor dragging. • Lack of assertiveness by the master once dragging was reported. Pilot advice at that time was to keep giving “Dead Slow Ahead” to maintain position. The key to regaining control was the need for positive and committed use of the Main Engine to reduce the weight on the cable, which was lacking. Increased power of the Main Engine was not utilized till too late. • Lack of Situational awareness by Bridge team. • Inadequate Risk assessment done for anchoring in congested anchorage with strong current. • Ship in the vicinity were not informed timely of the dragging of anchor, late notification given to Banglar Gaurab thereby giving inadequate time for them to prepare their engines.
Fouling of both the vessel’s anchor cables which worsened the situation and locked both the vessels together causing further damage. • Tripping of Power Pack when using the Bow Thruster. Use of the Bow Thruster could have possibly reduced the damage. • MV BANGLAR GOURAB WAS FOUND NOT MANNED PROPERLY at the time of our inspection. Deck department had one Master and two fresh cadets only when ENGINE department had one Third & one Fourth Engineer only.
